Community Health Supervisor
Responsibilities
- To act as liaison between formal health system and Community health worker
- To facilitate community health workers for mapping of un-covered area assigned
- To supervise and provide on the job training to CHWs, review and evaluate the performance of the CHWs.
- To Implement the community health services package devised in coordination with district government health departments of Lodhran and monitor its implementation
- To Guide the CHW’s in the implementation of the set action plans. Conduct monthly supervisory meeting with CHWs.
- To conduct regular visits to the CHWs in their communities to assess and assist in their work.
- To assist with the formation of community health committees.
- To identify and report immediately to the supervisor any problem that may interfere in achieving program objectives.
- To make Monthly Activity Report with CHWs and submit to supervisor.
- To collation and recording of data through Web based Management Information System (MIS)
- Any other task Assigned by Supervisor.
